The setting in ISPF option 0 (SETTINGS):

Tab to action bar choices

changes by itself sometimes. Is there any specific reason for this?

Or is it implicitly changed somehow by executing some different command?

Are you sure you are using the same profile dataset each time? Also, check if there were updates to the ISPF configuration table (done by the sys admin). You can checl the values of the config table with TSO ISPVCALL STATUS
